Reminders about Counseling Referral
(for instructors)

For Academic Referrals

• If the student needs or wants a tutor, they should be referred directly to Mary Schreiner in SC138 to avoid any delay. Please print this Tutoring Services Request, sign, and give to student to take to Mary Schreiner.

For Personal, behavioral or career referrals

• The more thorough the referral, the better we can assist the student
• Referrals with missing or incorrect information will be returned to instructors
  (Ex: phone number -missing or incorrect)
• Use one referral per student
• Turn-around time is affected by the student's response to phone calls, messages, and subsequent follow-up
• The information in this referral will be shared among the student, the instructor, and the counselor
